Most problems aren't unsolved, they're unseen. I spend the first phase finding what's actually blocking users, not what they say is blocking them. Usually those are different things.
Enterprise users are busy and context-switching constantly. If a flow needs explanation, it's not done yet.
Shipped is better than perfect, but shipping something nobody uses is worse than waiting. I push for small real releases so I'm always learning from actual behavior.
